*A $50 fee applies for prescriptions written outside of membership. Prescription fee is waived with General Derm Membership or Aesthetic Membership.

*A $75 prior authorization fee will apply if your insurance requires a prior authorization.

Membership carries a minimum of a *6-month commitment and a *90-day cancellation notice. Membership payments are automatically drafted monthly or quarterly and require a valid card on file. TFM does accept HSA/FSA funds.
